Abstract
Introduction: Cementoblastoma is a rare benign odontogenic tumour of ectomesenchymal origin. We present a case of a 21-year-old female with an 18-month history of persistent pain in the left posterior mandible. Method(s): Initial radiographic imaging revealed ill-defined radiolucent-radiopaque mixed density bony lesions, associated with a peripheral radiolucent halo, related to the lower left first molar roots and closely associated with the inferior alveolar canal. The preliminary radiographic diagnosis was cementoblastoma, which was confirmed histopathologically after an incisional biopsy. The lesion and symptomatic tooth were surgically excised. Follow-up imaging two years post-surgery showed stable but slow growth of the lesion. The patient reported increasing pain at the operative site. Result(s): Recurrence was confirmed three years postoperatively, with the tumour seen overlapping the inferior alveolar nerve radiographically. Conservative rim resection was performed without symptom relief, and progressive inferior alveolar nerve numbness was noted. Following further recurrence, a complete mandible resection was performed, with the defect supported by a reconstruction plate. Within 10 months, the defect was reconstructed with iliac crest cortico-cancellous bone graft. Conclusion(s): This report presents a rare case of recurrent benign cementoblastoma managed with progressive surgical interventions, culminating in a segmental mandibulectomy and two-stage bone graft reconstruction.Copyright © 2025
